You may register in three easy steps.

1.) Print out the appropriate attached registration form. Note: one for Boys and one for Girls.

• Boys PAPA Lax Registration form
• Girls PAPA Lax Registration form

2.) Fully Complete the form

3.) Return the form and a check made out to 'PAPA' by June 1st to:
